**Job Description:**

We are seeking a Home Health Registered Nurse to provide compassionate, patient•centered care in home settings. This role involves delivering high•quality nursing services and collaborating with healthcare teams.

**Key Responsibilities:**

• Provide nursing care in patients' homes.
• Use HCHB/OASIS for documentation.
• Develop and implement care plans with healthcare teams.
• Conduct assessments and monitor patient health.
• Educate patients and families on care procedures.
• Travel within Oakland, Wayne, and Macomb counties; mileage reimbursed.

**Requirements:**

• Active RN License and BLS Certification.
• 2+ years of home health RN experience.
• Proficient in HCHB/OASIS.
• Flexible hours; early mornings or late evenings possible.
• Reside within 30 minutes of the territory.

**Additional Information:**

• 13•week contract, 40 hours/week, no on•call or overtime.
• Weekly in•office presence required.
• Includes 2•3 days of orientation and team introduction.
