JOB DESCRIPTION

Director of Social Work

POSITION SUMMARY

The primary responsibility of the Social Worker is outlined in this description. The Social Worker is a professional position providing specialized services to individuals and families.

The Social Worker is expected to be self-directed, self-motivated, well-organized and must always display total professionalism in their attitude and appearance, in their relationship with individuals and co-workers, and completion of administrative duties.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Assist and work with the Executive/VP Chief Nursing Officer and guardians in developing end of life plans for all GHA Individuals with guardians/families..
  2. Coordinate applications, screenings, admissions, and assist as needed with discharges.
  3. Ensure application received/status letters are mailed to applicants and their guardian(s).
  4. Maintain GHA Referral Databases and individual applications.
  5. Maintain GHA’s admission’s phone line and email.
  6. Attend Individual Service Plan (ISP) or Individual Education Plan (IEP) meetings, as needed
  7. Attend SIS evaluation (Supports Intensity Scale) meetings, as needed.
  8. Assist families with guardianship.
  9. Attend with Family Network meetings.
  10. Communicate regularly with families and individuals.
  11. Complete abuse/neglect investigations.
  12. Assist Finance Department with Medicare/Medicaid eligibility.
  13. Monitor crisis/abuse/neglect on-call phone.
  14. Assist with and attend Individual Rights Committee meetings.
  15. Train to learn individual investigation procedures, inclusive of the North Carolina Incident Response Improvement System (IRIS).
  16. Maintain concern/complaints and their filing system on the Office 365, SharePoint Database.
  17. Remain current on state regulations and statutes.
  18. Encourage and maintain a positive work environment that promotes teamwork.
  19. Demonstrate a degree of independent judgment/action needed for problem solving activities.
  20. Work cooperatively with each GHA dept.
  21. Comply with all GHA policies.
  22. Inform supervisor of significant problems.

JOB QUALIFICATIONS AND REQUIREMENTS

Requires a master’s degree. Prefer certification or licensure in NC with two years’ experience and nonprofit experience. Valid Driver license. Knowledge of federal and state employment laws and regulations. Strong communication, public relations, and interpersonal skills. Extensive computer skills. Possess a strong ability for multi-tasking and leadership. Exhibits discretion and uses independent judgment. Professional oral and written communication skills. Ability to maintain confidential and sensitive information. Dissemination of confidential information; requires judgment and knowledge of policies. Must be able to hear, see, walk and run. Must be physically, mentally, and emotionally capable of performing the essential job duties. Ability to deal with stress. Regular and predictable attendance.

WORK ENVIRONMENT

This position may be site, office, community based. Employees in this position will be monitoring individuals with intellectual and developmental disabilities. There may be sudden and unexpected movements from the individuals the employee is monitoring. The environment may be loud at times. The position requires the employee to be on his or her feet most of the time. This environment requires outstanding time management skills and the ability to work independently to meet all aspects of making sure that the job is completed and that our “stakeholders” are satisfied (individuals, families, employees and outside stakeholders).

Training on the requirement of the HIPAA Privacy Rule and the plan’s health information Policies and Procedures under the direction of the plan’s Vice President/Chief Learning Officer is a prerequisite for this position. Employees who violate the requirements of the HIPAA Privacy Rule will be subject to discipline, up to including termination.
